(DE Driver\u2019s License or DE State ID)<\/li>\n\n\n\n\n   <\/ul>\n\n\n\n\n\n\n\n\n\n   <p>The <a href=\"medmarocpet.html\" title=\"Medical Marijuana Home Page\">Oversight Committee webpage<\/a> has details on how to request the addition of a new medical condition to the qualifying list.<\/p>\n\n\n\n\n   <strong>Steps to start the application process and required items needed:<\/strong>\n\n\n\n\n\n\n\n\n\n   <ul>\n\n\n\n\n    <li>Step 1 &#8211; Complete the Designated Caregiver Application\n\n\n\n\n    <ul>\n\n\n\n\n     <li>Complete the caregiver application in its entirety. You will need the patient\u2019s 10-digit registry number to apply. If you plan to be a designated caregiver for multiple patients, you must complete an application for each patient. <a href=\"\/wp-content\/uploads\/sites\/10\/dph\/pdf\/mmpcaregiverapplication.pdf\">Applications can be completed online or forms can be printed here.<\/a><\/li>\n\n\n\n\n    <\/ul>\n\n\n\n\n    <\/li>\n\n\n\n\n    <li>Step 2 &#8211; Complete the Patient Application\n\n\n\n\n    <ul>\n\n\n\n\n     <li>Complete the patient application in its entirety. You can apply online, or print a paper copy of the application. If you will require a caregiver, ensure that the caregiver completes a caregiver application. Information on how a caregiver can apply can be found here.<\/li>\n\n\n\n\n    <\/ul>\n\n\n\n\n    <\/li>\n\n\n\n\n    <li>Step 3 &#8211; Pay the non-refundable application fee\n\n\n\n\n    <ul>\n\n\n\n\n     <li>The fee is $50.00 and is non-refundable for all who apply. If you apply online, there will be a section for payment options. If you are mailing your application, please include a check or money order for $50.00, payable to the State of Delaware.<\/li>\n\n\n\n\n    <\/ul>\n\n\n\n\n    <\/li>\n\n\n\n\n    <li>Step 4 &#8211; Submit proof of age and residency\n\n\n\n\n    <ul>\n\n\n\n\n     <li>You must upload a copy of your Delaware issued driver\u2019s license or State issued ID with your application.
