You are still required to complete and submit the patient application. The only difference is your health care provider does not have to sign the application.<\/span><\/span><\/strong><\/h3>\n\n\n\n\n\n\n\n      <h3>Patient Applications<\/h3>\n\n\n\n\n\n\n\n      <p>In order to qualify for a standard Medical Marijuana card, you must meet the following qualifications:<\/p>\n\n\n\n\n\n\n\n      <ul>\n\n\n\n       <li class=\"dblspace\">Be 18 years of age or older or be a pediatric patient with parental consent.<\/li>\n\n\n\n       <li class=\"dblspace\">Have a Health Care Practitioner\u00a0Certification from a Delaware licensed MD, DO, APRN or PA.<\/li>\n\n\n\n       <li class=\"dblspace\">Be a Delaware resident with proof of residency. (DE Driver&#8217;s License or DE State ID)<\/li>\n\n\n\n      <\/ul>\n\n\n\n\n\n\n\n      <p>Steps to start the application process and required items needed:<\/p>\n\n\n\n\n\n\n\n      <ul>\n\n\n\n       <li>Step 1 &#8211; Health Care Practitioner Certification\n\n\n\n       <ul>\n\n\n\n        <li>The first step to getting a medical marijuana registry card in Delaware is to meet with your health care practitioner\u00a0about medical marijuana. Your health care practitioner\u00a0MUST be licensed to practice in Delaware and be a MD (Medical Doctor), DO (Doctor of Osteopathic medicine), APRN (Advanced Practice RN), or PA (Physican Assistant). Depending on if you apply online or via paper application, your health care practitioner\u00a0will complete a health care practitioner\u00a0certification if he or she feels that you will benefit from the use of medical marijuana.<\/li>\n\n\n\n       <\/ul>\n\n\n\n       <\/li>\n\n\n\n       <li>Step 2 &#8211; Complete the Patient Application\n\n\n\n       <ul>\n\n\n\n        <li>Complete the patient application, or pediatric patient application, in its entirety. You <a href=\"\/wp-content\/uploads\/sites\/10\/dph\/pdf\/mmponlineappinfo.pdf\">can apply online<\/a>, or print a paper copy of the application. If you will require a caregiver, ensure that the caregiver completes a caregiver application. Information on how a caregiver can apply can be found here.<\/li>\n\n\n\n       <\/ul>\n\n\n\n       <\/li>\n\n\n\n       <li>Step 3 &#8211; Pay the non-refundable application fee\n\n\n\n       <ul>\n\n\n\n        <li>The application fee is $50 for 1 year; $75 for 2 years; and $100 for 3 years which is non-refundable for all who apply.\u00a0 If you apply online, there will be a section for payment options.\u00a0 If you are mailing your application, please include a check or money order for the selected application fee, payable to the State of Delaware.<\/li>\n\n\n\n       <\/ul>\n\n\n\n       <\/li>\n\n\n\n       <li>Step 4 &#8211; Submit proof of age and residency\n\n\n\n       <ul>\n\n\n\n        <li>You, or parents or legal guardian for pediatric patients, must upload a copy of your Delaware issued driver\u2019s license or State issued ID with your application.
