An application can be initiated by contacting Central Intake at (302) 255-9800. Minimal\n\n\n\n\n\n\n  information is needed to start the application process such as name, address, phone number, and date of birth.\n\n\n\n\n\n\n<\/p>\n\n\n\n\n\n\n<p>\n\n\n\n\n\n\n  In order to determine eligibility, a copy of the most recent eye examination information must be provided.
