be given at least 30 days written notice of moves outside the facility; and be informed of your appeal and rights to live there again\r\n\r\n\r\n\r\n<\/li><li>Be fully informed by a physician of your medical condition; to be given the opportunity to participate in the planning of your medical treatment\r\n\r\n\r\n\r\n<\/li><li>Be free from chemical and physical restraint\r\n\r\n\r\n\r\n<\/li><li>Voice grievances without fear of reprisal\r\n\r\n\r\n\r\n<\/li><li>Send and receive personal mail\r\n\r\n\r\n\r\n<\/li><li>Be assured privacy for visits by your spouse or be permitted to share a room with your spouse if she\/he is also a resident\r\n\r\n\r\n\r\n<\/li><li>Associate and communicate privately with any person of their choice\r\n\r\n\r\n\r\n<\/li><li>Access to an Ombudsman\r\n\r\n\r\n\r\n<\/li><li>Have your records kept confidential\r\n\r\n\r\n\r\n<\/li><li>File a complaint or recommend changes in policies and services for yourself or others to the resident council, facility staff and\/or outside representatives to be free from restraint, interference, coercion, discrimination or reprisal; Each state has an Office of the State Long-Term Care Ombudsman (Office), headed by a full-time State Long-Term Care Ombudsman (Ombudsman) who directs the program statewide. Across the nation, staff and thousands of volunteers are designated by State Ombudsmen as representatives to directly serve residents. \r\n\r\n\r\n\r\n<\/p><p><u>\r\n\r\n\r\n\r\nOmbudsman is a Swedish word<\/u> that means advocate or representative of the people. In the Delaware Long-Term Care Ombudsman Program, Ombudsman representatives empower and support residents by: \r\n\r\n\r\n\r\n\r\n\r\n\r\n\r\n<\/p><div class=\"container\">\r\n\r\n\r\n\r\n<div class=\"row\">\r\n\r\n\r\n\r\n<div class=\"col-md-6\">      \r\n\r\n\r\n\r\n<ul>\r\n\r\n\r\n\r\n<li><span class=\"dhss-red\"><strong>Protecting<\/strong><\/span> residents rights <\/li>\r\n\r\n\r\n\r\n<li><span class=\"dhss-red\"><strong>Promoting<\/strong><\/span> dignity and choice<\/li>       \r\n\r\n\r\n\r\n<li><span class=\"dhss-red\"><strong>Meeting<\/strong><\/span> privately with residents <\/li>\r\n\r\n\r\n\r\n<li><span class=\"dhss-red\"><strong>Assisting<\/strong><\/span> residents to identify and resolve concerns <\/li>\r\n\r\n\r\n\r\n<li><span class=\"dhss-red\"><strong>Helping<\/strong><\/span> residents to file complaints and appeals<\/li> \r\n\r\n\r\n\r\n<li><span class=\"dhss-red\"><strong>Connecting<\/strong><\/span> residents to legal and community services <\/li>\r\n\r\n\r\n\r\n<li><span class=\"dhss-red\"><strong>Providing<\/strong><\/span> education on long-term care settings <\/li>\r\n\r\n\r\n\r\n<li><span class=\"dhss-red\"><strong>Advocating<\/strong><\/span> for quality care <\/li>\r\n\r\n\r\n\r\n<\/ul>\r\n\r\n\r\n\r\n<\/div>\r\n\r\n\r\n\r\n <div class=\"col-md-6\" style=\"border-style: solid;\">     \r\n\r\n\r\n\r\n      \r\n\r\n\r\n\r\n        <div id=\"feature-content\">    \r\n\r\n\r\n\r\n <br>\r\n\r\n\r\n\r\n<strong> Effective immediately!<\/strong> All long-term care facilities must allow visitation, including during an outbreak. On November 12th, 2021, the Centers for Medicare and Medicaid Services (CMS) updated its federal visitation guidance. There is no cost to residents or families for Ombudsman services.
