- Application for Child Support Services – (Check/Money Order are the only accepted methods of payment.)
- Affidavit of Lost or Stolen Check Form
- Change of Address Form (Must submit with photo ID.)
- Change of Name Form – Change of Name Form must be submitted with phot ID and preferably the Marriage Certificate, Divorce Decree, or Court order that changed their name.
- Direct Deposit or U.S. Bank ReliaCard Enrollment Form Direct Deposit must have supporting documentation of the bank account such as Voided Check or letter from the bank on the bank鈥檚 letterhead.
