What are home and community-based long term care services and where can I find them?
Home and community-based long term care services are provided to older persons and persons with disabilities in their own homes and communities as an alternative to institutional (nursing home) care. Examples of home and community based services include personal care, adult day services, respite, home-delivered meals, and many other types of care.
