Vendor/External Agency SFTP Access Forms

• The division requesting access should submit four copies of a completed Secure File Transfer Memorandum of Agreement (MOA) form, signed by the division and the outside entity.
  Secure File Transfer Memorandum of Agreement and includes:
  • Appendix A: DTI State Information Transport Network (SITN) Acceptable Use Policy. This is available at http://dti.delaware.gov/pdfs/pp/AcceptableUsePolicy.pdf
  • Appendix B (If the information transferred includes HIPAA-defined Protected Health Information (PHI))
    • HIPAA BAA template with the State as the Covered Entity(CE)
• DHSS Systems User Request Form For DB2, etc.
