For all veterinarians in Delaware: Delaware Monthly Rabies Surcharge
As a veterinarian, you are supporting the state spay & neuter fund and its mission to reduce pet overpopulation in Delaware. You do this by collecting and forwarding a rabies surcharge on each rabies vaccine administered to cats and dogs. 16 Del.C. �3014F
- Delaware Monthly Rabies Surcharge Remittance Report
This form is used by all veterinarians to submit the Delaware Monthly Rabies Surcharge Remittance Form. Veterinarians administering rabies inoculations in Delaware must collect a $3 surcharge for each vaccine and submit the collected surcharges along with all applicable rabies inoculation verifications to the Division of Public Health on a monthly basis. These surcharges fund the Spay & Neuter Program.

Spay & Neuter Program Surgery Provider Invoice Forms
- For-Profit Provider SNP Invoice for Certificates of Surgery
This form is for use by for-profit providers in the Spay & Neuter Program who have chosen to offer services to either income-eligible certificate holders or organization certificate holders.
- Non-Profit Provider SNP Invoice for Certificates of Surgery
This form is for use by non-profit providers in the Spay & Neuter Program who have chosen to offer services to either income-eligible certificate holders or organization certificate holders.
- Spay & Neuter Program Complications Invoice
Form to be used by all providers to report complications that occur during spay & neuter surgeries.
