    Primary Care Reform Collaborative

    The Delaware Health Care Commission’s Primary Care Reform Collaborative strengthens Delaware’s primary care system both by promoting efforts to reduce costs and increase access to high-quality and cost-efficient health care through value-based care delivery models. Two key priorities are to address a shortage of primary care providers and ensure commercial health plans reimburse primary care services at or above Medicare rates.
