    Delaware Medical Orders for Scope of Treatment (DMOST)

    For those with a serious illness, doctors may recommend a Delaware Medical Orders for Scope of Treatment (DMOST) form as part of their Advance Directive. This medical order gives patients, or their designated authorized representative, more control over specific life-sustaining emergency treatment preferences such as CPR and other medical interventions. The DMOST form applies to any health care setting – at home, by first responders, in the hospital or in a skilled nursing facility.

    The process encourages shared, informed medical decision-making between the patient (or authorized representative if the patient is not able to make decisions on their own behalf), and their doctor or advance practice provider (Advance Practice Registered Nurse, APRN, or Physician’s Assistant, PA).

    In some states, the form is called POLST, or Physician Orders for Life Sustaining Treatment.
