Purpose
The Chronic Renal Disease Program provides assistance to Delaware residents diagnosed with End Stage Renal Disease. The State of Delaware provides 100% of the funding for this program, which is administered by the Division of Medicaid and Medical Assistance. Because State funds are limited, all third-party resources (Medicare, Medicaid, VA, private insurance, etc.) must be considered before program funds are used.
Eligibility
You must meet certain medical and financial criteria to be eligible for support. Medically, you must be diagnosed with End Stage Renal Disease, receive dialysis, or have had a renal transplant.
Financially, your gross countable income must be below 300% of the Federal Poverty Level. If you have a legally married spouse, you will be considered as a household of two unless you are separated and maintain two separate residences for at least 12 months prior to application.
If you have Medicare Part A or Medicare Part B, you must apply for Medicare Part D Prescription Drug Coverage and, if eligible, for Social Security’s Extra Help Program, unless you have other insurance that provides the same level of prescription benefits.
Services Covered
The Chronic Renal Disease Program can provide payment for the unreimbursed cost of medications (prescription and over-the-counter) and nutritional supplements, including the cost of Medicare Part D prescription drug coverage. In addition, transportation may be provided to and from the dialysis unit, transplant center, or possibly related medical appointments.
Services Not Covered
The program cannot pay for insurance premiums (except for Medicare Part D premiums) or any medical services other than those described above.
How to Apply
- Call the Delaware Help Line at 1-800-464-4357 or 302-424-7180 and ask for the Chronic Renal Disease Program.
- Provide your mailing address or a fax number where we can send the application to you.
- Complete and return the application to the address on the form.
- We will contact you to schedule an appointment to determine if you are medically and financially eligible for the program.

The Delaware State Legislature established the Chronic Renal Disease Program, effective in 1970, by enacting Title 29, Chapter 79, Subchapter II of the Delaware Code.
