    Medicaid coverage for long term care, as well as home and community-based services

    Diamond State Health Plan Plus (DSHP-Plus, DSHP Plus, or DSHP+) is a mandatory managed care Medicaid program that provides long-term care services and supports for Delaware residents who are elderly or physically disabled. While nursing home facility care is one potential benefit, DSHP-Plus also covers a variety of Home and Community-Based Services to prevent and / or delay nursing home admissions. This might include adult day care, home meal delivery, personal care assistance, personal emergency response systems, and respite care.

    Delaware’s Division of Medicaid and Medical Assistance (DMMA) developed this integrated long-term care initiative in partnership with the state’s Division of Services for Aging and Adults with Physical Disabilities (DSAAPD) to:

    • Improve access to community-based long-term care services
    • More effectively address individual needs
    • Better control rising long-term care costs that significantly impact Medicaid.

    Diamond State Health Plan Plus 1115 Demonstration Waiver Amendment

    1115 Demonstration Waivers give states additional flexibility to design and improve their programs to demonstrate and evaluate state-specific policy approaches to better serving Medicaid populations.
