The Division of Services for Aging and Adults with Physical Disabilities (DSAAPD) connects older adults, individuals with physical disabilities, and their caregivers with vital programs, services, and support systems throughout Delaware. We are here to help you navigate options, access care, and find solutions that enhance independence, safety, and quality of life.

What We Do for You
- Design care plans – DSAAPD case managers will help assess your unique situation, develop care plans to meet your needs, and provide ongoing coordination to help make sure you stay connected with the services you need.
- Support Caregivers – We offer information, training, and services to help caregivers—including grandparents and relatives raising children—manage responsibilities and avoid burnout.
- Promote Healthy Living – We partner with local organizations to offer wellness programs, nutrition support, and chronic disease self-management tools for older adults and people with physical disabilities.
- Expand Access to Services – From transportation assistance to home modifications, we help people live independently and access the care they need.
- Develop and Improve Programs – Based on community needs and feedback, we design and support services that make a meaningful impact across the state.
- Raise Awareness and Advocate – We work to ensure that the voices of older adults, individuals with disabilities, and caregivers are heard—pushing for inclusive policies and expanded resources.
DSAAPD also operates the Delaware Hospital for the Chronically Ill (DHCI), a skilled nursing facility located in Smyrna, providing high-quality long-term care to individuals with complex health needs.
