    Transition Age Youth Program

    The Division of Substance Abuse and Mental Health (DSAMH) offers a Transition Age Youth program to connect young people ages 16-24 who are currently engaged with a youth provider for mental health and/or substance use disorder with services from adult providers. Dedicated case managers provide case management support, specific to client needs, to alleviate gaps in care in the transition from youth to adult provider.

    Youth providers should work with clients to complete the Transitional Age Youth Brief Screen and send it to: DSAMH_TAY@Delaware.gov.

    Youth, or their parent or legal guardian if under 18, must also complete the Transitional Age Youth Release of Information form and return it to the Division of Substance Abuse and Mental Health, ATTN: Transition Age Youth Program, 1901 N. DuPont Highway, New Castle, DE 19720.
