    HCBS Providers

    A robust network of qualified providers deliver supports and services under the Division of Developmental Disabilities Services (DDDS) Medicaid Home and Community-Based Services (HCBS) Waiver. As required by Medicaid regulations, DDDS accepts applications on an open and continuous basis.

    Follow these steps to become a Delaware HCBS provider:

    Phase 1A – Information Gathering

    To begin the process of becoming a provider in Delaware, review the Provider Application Manual in its entirety as well as other helpful resources.

    Phase 1B – Information Gathering Session

    After reviewing the appropriate information above, applicants should listen (“attend”) to the following sessions. This will complete phase 1.

    Phase 2 – Submit Application

    All application and supporting document submissions for new services with DDDS must be sent to DDDS_ProviderAuthCommittee@delaware.gov. Prospective providers should review page 5 of the Provider Application Manual and complete the appropriate resources below to submit a successful application. Incomplete applications will not be accepted.
