    For Home and Community Based Services

    Division of Developmental Disabilities Services (DDDS) clients may receive Home and Community-Based services under the Medicaid Lifespan Waiver, or through their Diamond State Health Plan Plus Medicaid managed care organization.

    It is important to understand that an application to DDDS is not an application for Medicaid waiver services. You must apply for Medicaid coverage separately. Your case manager will help you determine whether the Lifespan Waiver or Diamond State Health Plan will better meet your individual needs.

    The Lifespan Waiver provides eligible Delawareans access to Medicaid-approved services through the 1915(c) Home and Community Based Services Waiver. The Lifespan Waiver supports individuals to live more independently in their homes and communities through Home and Community Based Services that promote and support community living.

    Delaware’s State Medicaid Plan, Diamond State Health Plan Plus, also covers Home and Community Based Services, especially those that could help prevent or delay admission to a long-term care facility. These services might include adult day care, home meal delivery, personal care assistance, personal emergency response systems and respite care.

    It is important to understand what makes the Lifespan Waiver and Diamond State Health Plan Plus different to help you decide what services under which plan will be best for you right now in your life. Even if you meet the criteria to get services under both programs, you must choose one. You cannot get services from both at the same time.
