    If you or someone you know is experiencing vision loss, the Division for the Visually Impaired (DVI) is here to help. Applying for services is simple—and we’ll guide you through every step. You can start the process yourself or refer someone else who may need support.

    Start Your Application

    1. Contact Central Intake
      Call (302) 255-9800 to speak with our Central Intake Coordinator.
    2. Fill out an Eye Report Form
      This form includes information from your eye doctor that helps us understand your level of vision.

    3. Provide a few basic details:
      • Your name
      • Your address
      • Your phone number
      • Your date of birth

    Once we have your information and the completed Eye Report, we’ll review your eligibility and connect you with the services that best fit your needs.
